Mingus Union High School students and faculty will receive H1N1 flu vaccinations Nov. 5 in the small gymnasium.
Service will not be denied, and no student payment is required.
Students who are covered by insurance are asked to bring copies of their insurance cards and other insurance information.
All students must bring a consent form signed by a parent or guardian. No student will be vaccinated without a signed consent form.
Both injections and nasal vaccinations will be available, but the school cannot guarantee which form of the vaccination each student will receive.
The vaccination program will be provided by the Yavapai County Health Department.
An information packet sent home to the families of all students states that high school students need to have only one vaccination.
All necessary insurance, consent and information forms are included in the packet.
Clarkdale-Jerome School students also will be vaccinated on campus Nov. 5.
Superintendent Kathleen Fleenor said students were taking home permission slips Oct. 22.
